AdventHealth and Aidin Collaborate to Enhance Care Coordination

New York, NY February 11, 2025  – AdventHealth, a connected system of care for every stage of life and health with more than 50 hospitals across nine states, has engaged Aidin as part of a system-wide initiative to improve the patient experience and the pace and quality of care. Aidin’s pioneering solution delivers workflows and automated processes to enable AdventHealth’s Care Management team to more efficiently and effectively oversee each patient’s personalized care transition plan.

“Aidin’s platform will allow us to standardize workflows to reduce administrative burden and decrease variability in care transitions across the continuum of care,” said Jeannine Nylaan, MSN, RN, Executive Director of Care Management at AdventHealth. “We have a responsibility to deliver care that is tailored to each individual’s physical, emotional, and spiritual needs. Having the right people, processes, and technology in place is critical to deliver on our commitment to treat the whole person. This is a big step in our journey to transform care coordination for our patients, enabling our team to facilitate timely progression of care throughout the care continuum.”

According to a study by McKinsey & Company, 90 million lives will be in value-based care models by 2027, up from 43 million in 2022. As healthcare organizations navigate the ongoing shift to value-based care, it is imperative to implement a system-wide approach to care management, closing gaps, and creating greater alignment among all stakeholders.

Through this partnership, AdventHealth’s more than 50 hospitals will leverage Aidin’s technology and expertise to provide patients with the best possible care across the healthcare continuum.

AdventHealth’s expansive Care Management team will leverage the Aidin platform to:

  • Reduce length of stay (LOS) and readmissions
  • Increase hospital bed capacity
  • Make it simpler to identify the most clinically appropriate, highest-quality post-acute setting for each patient
  • Reduce administrative burden so care managers can spend more time with patients
  • Enhance collaboration and communication with payors and post-acute care providers
  • Avoid delays associated with prior authorizations
  • Minimize operational costs associated with inadequate transitions of care
  • Streamline CMS (Centers for Medicare and Medicaid Services) compliance needs, including IMM (Important Medicare Message) and MOON (Medicare Outpatient Observation Notice) forms
  • Enhance coordination of emergency and non-emergency transportation

“As AdventHealth continues to grow and serve more communities across the country, we’re proud to support its Care Management team in their ability to discharge patients on time, every time, and to the highest quality provider,” said Russ Graney, Aidin’s Founder and Chief Executive Officer. “Aidin helps realize this vision by providing unrivaled access to data and insights that can be leveraged to assess each patient for their unique needs, provide better quality choices for post-acute care, and help patients transition on-time.”
